Liability Waiver for Qi Gong & Meditation Classes
Participants in Qi Gong or Meditation classes acknowledge the physical nature of these activities and assume responsibility for any risks. This waiver outlines participant responsibilities and releases Infinite Vitality from liability.

By providing your cell phone number, you consent to receive appointment reminders, confirmations, and practice updates via text message. Standard messaging rates may apply.

Consent forms for acupuncture and Eastern medicine treatments explain procedures, potential risks, and patient rights. These forms are required before beginning treatment.
Pricing, Practice and Insurance Policies
Cancellation Policy
Client Cancellations
If a client cancels on the same calendar day as their appointment, they will be charged for the full session.
Example: If your appointment is scheduled for October 12th, you may cancel on October 11th at 11:59 PM without penalty. However, if you cancel on October 12th at 12:00 AM or later, you will be charged for the full session.
Practitioner Cancellations
If Dr. Leigh must cancel on the same calendar day as your appointment, any prepayment will be fully refunded AND you will receive one complimentary session of equal value as our apology for the inconvenience.
Lateness Policy
For Short Sessions (45 minutes):
- If you arrive more than 15 minutes late, the session will be cancelled and you will be charged for the full session.
For Long Sessions (90 minutes):
- If you arrive over 30 minutes late, the session becomes a short session (45 minutes), but you will still be charged the long session price. We will not be able to complete all the work of a Structural Integration session.
- If you arrive more than 45 minutes late, the session will be cancelled and you will be charged for the full session.
What to Wear
For Short Sessions (45 minutes - Acupuncture/Craniosacral):
- Come as you are or wear loose, comfortable clothing similar to what you would wear for exercise.
- Tight clothing is not appropriate as it restricts access to treatment areas.
- For acupuncture sessions, Dr. Leigh needs to be able to access your knees.
For Long Sessions (90 minutes - Structural Integration/Bodywork):
- Wear an appropriate swimsuit or underwear that allows full access to the back line and unobstructed access to the legs.
- Tube tops are provided for women who prefer them for the session.
Insurance Coverage
Infinite Vitality accepts the following insurance types:
- PPO Plans
- Auto Accident (PIP - Personal Injury Protection)
IMPORTANT: You must complete a phone consultation to review your insurance coverage before scheduling your first appointment. If you schedule without this consultation, you will be charged the time-of-service price for your session.
IMPORTANT: You are responsible for the entire "billed amount" of your insurance appointment. Make sure you check your deductible amounts for "Out of Network Provider". You will be billed directly for the entire billed amount that your insurance does not pay.
Pricing Information
All displayed prices reflect a 40% discount from standard billed rates.
Example: A standard 90-minute session is billed to insurance at $350. The discounted time-of-service price is $210.

- Infinite Vitality is located inside Suite B
